To obtain a copy of your claims record - also called claims history statement - simply download and complete the form Claims History Statement, Verification and/or Rectification Request.
Remember to sign your request and include a legible photocopy of your driver's licence.
You may also consult your file at the office of the Groupement des assureurs automobiles on weekdays between 8:30%nbsp;a.m. and 4:30 p.m. You will then have to show your original driver's licence.

Think there's a mistake in your file? Ask the the Groupement des assureurs automobiles to verify the information it contains. If applicable, corrections will be made to your file.
If you already have a copy of your claims history statement, please contact the information officer whose name appears on the statement. If you do not have your claims history statement, you should:
- Write a letter bearing your original signature and showing your name, address and telephone number or fill out the form to that effect;
- Specify the information to be checked;
- Attach a copy of your driver's licence in which your name, address, licence number and signature are legible.
You will receive an acknowledgement of receipt and a reply will be sent to you within 21 days.
